Billing Follow-Up: What Every Useful Claim Note Should Include

By Vital Revenue Solutions • October 12, 2026

A useful claim note should let another team member understand what happened and continue the work without starting over. Consistent notes support clearer handoffs, especially when several people work the same account.

           Capture the current claim status

Record the date of the inquiry, how the status was checked, and what the payer reported. Distinguish between a claim that was received, a claim still being reviewed, a request for information, and a completed payment or denial. Do not assume that successful submission means payment has been approved.

Document the evidence and action taken

Include a portal confirmation or call reference number when available. Note the information reviewed, any documents requested, and what your team submitted or corrected. Link supporting records within your authorized system so the work can be traced.

           Write a specific next action

“Called payer” describes activity. “Payer requested the missing attachment; billing owner will submit it through the approved portal and recheck status on the recorded follow-up date” explains what needs to happen. Assign an owner and a date for that next action.

           Keep unresolved items visible

Use a shared worklist or task queue to show accounts waiting for a payer response, internal information, or a review decision. A useful handoff identifies the obstacle and the person responsible for moving it forward.

           Review notes for quality

Periodically check whether notes contain a status, supporting reference, action, owner, and follow-up date. Look for accounts with repeated activity but no progress. Those accounts may need escalation or a different research step.

            A simple note structure:

Date and channel → current status → evidence or reference → action completed → next action → owner → follow-up date.

Medicare providers can review available inquiry methods in

. Follow the relevant payer’s instructions when working each account.

Denial Tracking: Turn Repeated Problems Into Clear Next Steps

By Vital Revenue Solutions • October 12, 2026

A denial log is most useful when it supports two tasks: resolving the individual claim and understanding why the same problem keeps returning. A list of denial counts alone does not explain the next step.

           Read the payer explanation first

Review the remittance advice and the available payer explanation before deciding whether the claim needs correction, additional information, or an appeal. Claim adjustment reason codes and remittance advice remark codes help explain adjustments. Read them together with the claim details and applicable payer instructions.

           Build a consistent denial log

Capture the payer, claim reference, denial date, reason and remark codes, balance, deadline, assigned owner, corrective action, and follow-up date. Use broad operational categories such as eligibility, authorization, missing information, submission issues, or documentation review, while preserving the payer’s exact explanation.

Connect the problem to the responsible workflow

If the same issue appears repeatedly, review where the needed information enters the billing process. A handoff between scheduling, registration, clinical documentation, coding, and billing may need a clearer checklist or owner. Share patterns with the people who can address them.

           Track outcomes, not just resubmissions

Document what changed, when a response was sent, and the result. Follow the payer’s instructions for corrected claims and appeals. Avoid repeatedly sending the same information without understanding why the prior response did not resolve the issue.

Choose one improvement at a time

Start with a recurring category, agree on a practical process change, and review whether new claims show the same issue. This makes your denial review a tool for improving the workflow as well as working outstanding balances.

A/R Cleanup: Build a Worklist That Moves Claims Forward

By Vital Revenue Solutions • October 12, 2026

An aging report tells you what is outstanding. A useful worklist tells your team what to do next. Start A/R cleanup by turning balances into specific, assigned actions.

           1. Confirm the information before prioritizing

Review the date of service, payer, claim number, balance, last submission date, and current claim status. Reconcile posted payments and adjustments before treating a balance as unpaid. An old balance may need research, a correction, or an accounting review rather than another submission.

           2. Group accounts by the work they need

Separate claims awaiting a payer response from denials, missing information, possible underpayments, and balances needing internal review. Within each group, consider age, amount, the applicable payer deadlines, and how ready the account is for action.

A practical worklist includes: account reference, payer, claim status, outstanding balance, last action, assigned owner, next action, and follow-up date. Keep patient information in your authorized practice systems.

           3. Give every account an owner and a next step

Replace notes such as “follow up” with a specific task: confirm receipt in the payer portal, obtain the remittance advice, review a denial, or gather missing documentation. Record the result and any reference number so the next person can continue the work.

           4. Review progress consistently

Set a regular review of completed actions, unresolved balances, missed follow-up dates, and recurring obstacles. Track resolution as well as activity. Repeated calls without a change in claim status should prompt a different approach.

           Start with a manageable group

Choose one payer or aging category, use a consistent process, and refine the worklist before expanding it. Clear ownership and useful notes make a backlog easier to understand and manage.

. Confirm requirements and deadlines with the payer responsible for each claim.
